Activate a suggestion page

Hi,
I would like to create a simple suggestion page.
I have made the page in FrontPage (the form):
It has 3 empty fields that has to filled in by the visitors and a submit button.

__________________
Suggestions:

Now, the problem is that I have no idea how to make it functional. So far itâ€™s just a dead page.How can I bind it to an e-mail address or something like this?
