Creatign a Referral Form to put into a site

I downloaded a zip file from PHPForm.org which are the files I guess that are needed to put a Referral Form out on my website, but not too familiar with .js files or css files.

Any help would be greatly appreciated.

".js" files are the JavaScript files that will be used. The ".css" file is the presentation or styling file which you use to style the form to match the theme of your site.
