Converting HTML to PDF in a webform

I am looking for code or software that will allow me to convert a webform into a PDF. I need the PDF to be formatted with a company header.

I have included a link of site that is exactly what I want to do (different content). I need users to be able to upload photos and complete a webform.

Here is a sample of what I would like to do.

Web Form

Hi,

You can use PDF Duo .NET component in you ASP.NET application.

This component can create pdf of any forms as you like. For example, you can compose a html string of you report and using PDF Duo .NET convert it into a pdf.

I think it is very easy and flexible way for you. Shocked

The Adobe PDF document will be converted and returned to you in your browser application. Depending on the complexity of the document and the number of requests being made on the server at the time,the conversion process may take a while.
